Usage:

 

Data entered in this section is not necessarily very important and in no way binding, yet can be placed to have a better picture of an existing customer.

There are five sections (tabs) that can be accessed on the bottom of the form. All these sections are to maintain additional details of the existing customer such as: practice doctors, affiliations, staff, and other phone numbers or contacts.
